By checking this box, I, the parent/legal guardian as stated above do hereby consent to the above learner undertaking the trip and participating in the MEPS Excursion to Gauteng (UJ Arts Theatre and Sci-Bono Discovery Centre) on 06 May 2025.

I confirm that -

·    I also give permission for him/her to participate in all related activities planned by the school.

·    I also give consent for him/her to be under the care of the MEPS staff member(s) for the duration of the excursion, with the assurance that there will be strict supervision.

·    I hereby authorise the MEPS staff member(s) in charge to make arrangements for the welfare of my child (including medical or surgical treatment when I am out of reach) in case of an emergency.

·    I give my consent for my child to be transported by ambulance/private vehicle driven by a teacher, parent or another person as the case may be, in case of an emergency.

·    I, furthermore, indemnify and absolve the aforesaid school, Board of Governors, its staff, participating parents and Myngenoegen English Private School against all claims of whatsoever nature and howsoever arising directly or indirectly out of the child’s participation in the said educational excursions.

·    I give this undertaking on behalf of myself and my child in the knowledge that the Principal and staff will nevertheless take all reasonable precautions, for the safety and welfare of my child.
